"Side by Side With You"

C.A.M.P. is our Community Aging Management Program providing a continuum of care and services, on a fee for service basis, for persons and their caregiver living at home.

C.A.M.P. serves people with chronic conditions of aging and illness, including Alzheimer's and other dementias helping them to remain in their own homes, if desired and appropriate, for as long as possible.

The goal of the program is to help clients anticipate the progression of their disease, avert crisis and maintain as much choice and control as the situation will allow.

We will be side by side with you throughout the course of the disease.

Through nursing case management, education, and support, C.A.M.P. will help you keep the quality of your family life intact. C.A.M.P. will link you to many providers offering medical, psychological, home health, and other services to help you. Most importantly, nursing support will be available, when you need it in a crisis, 24 hours a day by telephone.

C.A.M.P. clients can benefit from the support necessary to remain at home, however our services can be provided in any setting residential or clinical.

Services Include:

• Initial and Ongoing Assessment
• Ongoing Case Management
• Create linkages between other service providers and clients if needed
• Maintain ongoing contact with clients and families and update care plans as needed
• 24-hour Help Line
• Helping arrange for and overall supervision of staffing
