| What is C.A.M.P.?
C.A.M.P. serves people with dementia and helps them
to remain in their own homes. C.A.M.P. is a Comprehensive
Alzheimer's Management Program providing a continuum of care
and services for persons with Alzheimer's and their caregiver
living at home. |

The goal of the program is to help people anticipate the progression
of the dementia, avert crisis and maintain as much choice
and control as the situation will allow.
We will be side by side with you throughout the course of
the disease.
Why Call C.A.M.P. ?
A diagnosis of Alzheimer's can be devastating. It doesn't
have to be. Alzheimer's is a tough disease. It is especially
difficult to cope with alone.

C.A.M.P. was created to be with you as you face the
challenges Alzheimer's presents.
Through case management, education, and support, C.A.M.P.
will help you keep the quality of your family life intact.
C.A.M.P. will link you to many providers offering medical,
psychological, home health, financial and other services to
help you. Most importantly, support will be available, should
you need it, 24 hours a day by telephone.
C.A.M.P. clients can benefit from the support necessary
to remain at home, however our services can be provided in
any setting residential or institutional.
