The Resident Care Services program meets the differing needs of members of The Coves by providing supportive services on an individual basis. Resident Care Services at The Coves offers a flexible nursing care management program to give you and your family peace of mind.

Resident Care Services at The Coves provides assistive services that can be scheduled and are routine, allowing someone to remain independent in their own home. A licensed nurse case manager oversees care and certified nursing assistants are available for personal care and comfort.

Resident Care Services at The Coves was developed specifically to assist members of The Coves maintain their independence. This program provides broad-based, multidisciplinary support services to members of The Coves on an individual basis.

Our goal is to always provide a supportive environment offering on-premises services which are personalized, appropriate and affordable. Sometimes just a little help keeps a person independent for a long time.

Services Include:

• Symptom/Condition Review
• Medication Reviews
• Mental Status & Sensory Exam
• Functional Assessment
• Evaluation of Living Arrangements and Support System (including home safety)
• Development of Care Plan
• Ongoing assessment to determine medical stability
• Medication Management
• Create links between service providers and clients
• Maintain ongoing contact with clients and families
• 24-hour On-Call Availability
